Practice Champions are patients who have been trained and are getting involved on a voluntary basis and run different activities for the practice.

One of our patients is supporting the Conversation Group at Park Lane Medical Centre if anyone else is interested in helping out they were welcome to go along

On Tuesday 29th April 2015 a Celebration event was held at the Great Victoria Hotel in Bradford. The Lady Mayoress Mrs Elizabeth Sharp attended and this was an event where the champions were thanked by the practices for their contribution to the community.

Information for Patients
What is a Patient Group? The aim of a group is to get patients involved with the practice in a positive way and to help the practice provide services that patients want. Involving patients helps the practice understand patients needs from a patient perspective and therefore helps the practice with decision making.
When did it start? Our first Meeting was on 30th June 2011
Who is involved from the practice? The whole primary healthcare team is involved in different ways, usually this will be with Angela Worobel, Practice Manager, and various other members of the team, e.g. GP’s, Practice Nurses, Receptionists etc.
Who else is involved? The practice works in partnership with NHS Bradford & Airedale and different representatives may be invited to make presentations to patients or make requests to use this forum for consultation purposes when re-designing services. NHS Bradford & Airedale is responsible for local health services.
What is discussed at the meetings? The agenda is driven by particular topics that the practice wish to bring to the attention of patients. Patients also contribute to the by raising anything they wish. Usually items that can be discussed are: appointments, telephone access, new services coming on line, alterations to premises and services, practice leaflet. Meetings are usually monthly
Do I have to attend each meeting? No you do not have to attend each meeting. Once we know that you would like to become involved you can decide for yourself exactly how much you would like to become involved. If you would just like to receive the minutes of the meetings then you can pass on your contact details.
Other ways to get involved? If you don’t have time to attend meetings then you can get involved by agreeing to become a “virtual member” by agreeing for us to contact you from time to time by e-mail

The aim of the PPG is to ensure that effective communications are established in the Practice for all service users and staff. The PPG will be the arena where issues and concerns are raised by participants in order to influence and develop healthcare services with particular emphasis on Horton Park Surgery.
1. To make a positive difference.
2. The PPG shall, from time to time, allocate agreed tasks to working groups and/or (sub) committees. The Remits, Terms of Reference, modes of report, timescales etc shall be determined by the PPG at the same time.
3. To be actively involved and consulted in any potential service developments.
4. Provide feedback on patients’ needs, concerns and interests.
5. Continue to improve communications.
6. Receive support from health professionals to achieve shared goals and to access relevant training.
7. Work closely with professionals on key issues.
8. Initiate, help and attend events to promote and educate on health issues.
9. Initiate aid in the design of and evaluate the outcomes of surveys.
10. Liaise with other Patient Participation Groups in the area (NHS Bradford & Airedale can help put you in touch with other groups).
11. Affiliate to NSPP (National Association of Patient Participation) in order to gain support and resources.

The Chair / Vice ChairThe Chair will be a patient elected annually by the PPG group members by ballot and can serve a maximum of 3 continuous years.The Chair’s responsibilities being involved with the agenda for group meetings, advising on issues relating to the meetings and follow up of issues arising from meetings.
The PPG will be meeting as agreed. The venue for the moment will be Clinic Room at Horton Park Surgery.
Papers will arrive no less than one week prior to the meeting date.
