Welcome to Horton Park Medical Practice
To register with the practice you can register online using the following link – https://gp-registration.nhs.uk/B83035
If you do not have access to the internet, you can attend the surgery in person. If you have an NHS number, please bring it with you. You will be asked to complete a form and bring a proof of identification (Passport or Driving License) and proof of address (Bank Statement or Utility Bill).
It usually takes up to a week to process new registrations.
We will only register you if you live within our practice area shown below.
