Non-NHS Services
The NHS does not cover the cost of completing non-NHS reports, forms and letters, such as reports requested by insurance companies. In accordance with British Medical Association (BMA) guidelines, a fee will therefore be charged for these services.
- Please note that completing these requests is not a clinical priority for the practice and may take 4–6 weeks.
- To request a report, form or letter, please submit your request in writing to Reception, together with any relevant forms that need to be completed.
- Please print and complete the form below, or come into the surgery to collect a copy of the form. – Non NHS work Form 2026
- You will be contacted by the Secretary Team when your letter or report is ready for collection.
The practice do not do letters for first PIP claims only PIP appeals (you can request a patient summary), universal credit (you will need a sick note).
The doctors will not countersign passport applications and letters for schools.
